Free Prize Draw

This Competition applies only to promotional packs of Macleans toothpaste marked with instructions on how to win "A £5000 makeover for you and your friend with thanks to Dancing on Ice".

Restrictions on Entry

This promotion commences on 02/01/2012 and entry to the weekly draws close on 23.59 on 25/03/2012, ("Weekly Closing Date") with the final date for entries into the final draw being 23.59 on 20/04/2012 ("Final Closing Date")

Routes of Entry

Entry is open to UK residents aged 18 or over, excludes ROI. No bulk, third party or organised group entries will be accepted.

Purchase any promotional Dancing On Ice pack of Macleans and find the B Code (B followed by 6 digits) on the inside of the outer packaging then either:
  • Send a text message to 62277 with the keyword DOI, followed by your B code and your name (eg DOI B123456 John Smith); OR
  • Send your name, full address and contact number together with the promotional pack to "Macleans Dancing on Ice, PO Box 101, Leigh, Lancs, WN7 1WQ to arrive before 20 April 2012.

For legal reasons NORTHERN IRELAND residents only may enter without a purchase by sending their name, address and contact number to "Macleans Dancing on Ice NPN" PO Box 101, Leight, Lancs, WN7 1WQ to arrive before 20 April 2012.

Maximum of three entries per B code per person during the promotion period (and the same number of NPN requests for NI)

Code Location: B code (B followed by 6 digits) found on the inside of the outer packaging of pack

For SMS entrants: SMS entrants should retain their packaging (specifically the section containing the B code) as it might be required to validate a prize claim. Standard text rates apply, typically 10p but may be part of free text packages. Entrants will receive a return text message (free of charge) confirming their entry and advising how to opt-out of further communications.

Prizes

All entries will be entered into the next available and all subsequent weekly prize draws as well as the final prize draw.

TWELVE WEEKLY DRAWS for a mini makeover: Each weekly period runs from 00.00.01 on the Monday until 23.59 the next Sunday. . Maximum one weekly prize per person.

Weekly prize includes: 1 x mini makeover for two people for winner plus guest with a choice of three options: (1) Hair makeover; (2) Aromatherapy massage package; OR (3) Revitalizing facial package. Prize includes one nights’ accommodation at a 4* hotel, based on two people sharing a room, and includes breakfast, three course dinner with a glass of wine. Winner and guest to choose the same option. Both the winner and guest will receive all package benefits of the option selected. The promoter will supply a voucher for £75 per person to be used towards travel. Travel costs above this ceiling, and all other costs not specifically set out will be the responsibility of the winner.

ONE FINAL PRIZE DRAW: All valid entries received during the promotional period up until 20/04/2012 will be entered into a separate draw and one will be will be selected.

FINAL DRAW PRIZE: 1 x makeover for two people worth £5,000 for winner and guest. Includes two nights’ accommodation at a 4* hotel, based on two people sharing a room, and includes breakfast and one three course dinner with a glass of wine, professional photography shoot, professional make up application, hair styling, spa treatment, personal shopper, £500 worth of clothing vouchers, and 8 x 1 hour (two per week) personal training session ahead of the shoot. The winner and guest will both receive all benefits of prize as listed, with a combined total value of £5000. The Promoter will supply a voucher for £50 per person to be used towards travel. Travel costs above this ceiling and other costs will be the responsibility of the winner.

All prizes must be booked at least 14 days in advance, are subject to availability and must be confirmed by 31/05/2012.

Winner and any guest will be required to provide proof of age and identity.

Once a booking has been confirmed no changes are permitted. If a booking is cancelled no alternative dates will be issued. For the avoidance of doubt, no compensation and/or refund will be provided.

Damaged or photocopied proof of purchase or B-codes will not be accepted.

No compensation will be payable if a winner is unable to use the prize as stated.

The Promoter is not liable for third party products or services and all elements of the prizes are subject to terms and conditions of the relevant supplier.

The winner (and where applicable their guest)is responsible for obtaining medical advice before starting any exercise programme.

GSK GENERAL COMPETITION TERMS AND CONDITIONS
  1. These terms and conditions together with any specific rules set out in Competition Notices (as defined below) are the Competition Rules ("Rules") and apply to competitions conducted by GSK Consumer Healthcare in the UK ("GSK") including online competitions at www.macleans.co.uk "Sites" organised by GSK (or a third party on GSK’s behalf) ("Competitions"), unless otherwise expressly stated. By entering a Competition, entrants agree to be bound by these Rules.
  2. The Competition is organised by GlaxoSmithKline Consumer Healthcare of 980 Great West Road, Brentford, Middlesex TW8 9GS ("GSK").
  3. Rules specific to each Competition are displayed in a notice relating to each individual Competition ("Competition Notice") and are incorporated into the Rules. In the event of differences or discrepancy between these terms and conditions and the Competition Notice, the Competition Notice shall prevail.
  4. GSK reserves the right to cancel or amend the Competition or the Rules without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside GSK’s reasonable control. Any changes will be posted either within these terms and conditions or the Competition Notice. A copy of the Rules may also be obtained by sending a stamped addressed envelope to: Macleans Competition Rules, GlaxoSmithKline Consumer Healthcare, 980 Great West Road, Middlesex TW8 9GS .
  5. In the event of any dispute regarding the Rules, conduct, results and all other matters relating to a Competition, the decision of GSK shall be final and no correspondence or discussion shall be entered into.

Eligibility
  1. Employees (or members of the families) of GSK or any company involved in the Competition are not eligible to enter the Competition. GSK reserves the right not to award a prize (and to select an alternative winner) if GSK is aware or has reasonable grounds to believe that a winner is not eligible.
  2. Additional eligibility requirements may apply to a specific Competition. These will be set out in the Competition Notice.
  3. By entering the Competition, you hereby warrant that all information submitted by you is true, current and complete. GSK reserves the right to verify the eligibility of all entrants.
  4. The address you provide with your competition entry ("Entry") will be used to send any prizes. Unless stated otherwise in the Competition Notice, Competitions are only open to residents of the United Kingdom and you are not entitled to enter the Competition if you are resident outside of the United Kingdom.
  5. Where a Competition is open to all age groups, GSK assumes that by using the website and entering the Competition (and you warrant that) you are aged 18 or over or, if you are under 18, that your parents have consented to your entry into the Competition and these Rules. If a Competition is only open to a certain age group (e.g. 18 and over), this will be set out in the Competition Notice and GSK assumes that by entering the Competition (and you warrant that) you are the appropriate age to enter the Competition.
  6. GSK reserves the right to disqualify any entrant if it has reasonable grounds to believe the entrant has breached any of the Rules.

Eligibility
  1. Unless stated otherwise in the Competition Notice, you may enter the Competition a maximum of three times per person using the B code on pack. Should the winner selected not be contactable after reasonable efforts or who fail to return contact within a further 14 days will be disqualified and a reserve entrant will be selected on the same basis.
  2. Competition entries must be made in the manner and by the closing date and time specified on the Competition Notice. Failure to do so will disqualify the entry.
  3. Where an offer is made for participation in a Competition involving a premium rate telephone call, the Competition Notice will include details of the anticipated cost, and any other guidance to which GSK must adhere to.
  4. Proof of posting or emailing cannot be accepted as proof of delivery. GSK cannot accept responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft, destruction, alteration of or unauthorised access to Entries, or Entries lost, damaged or delayed as a result of server functions, technical issues, virus, bugs or other causes outside GSK ’s control.
  5. Entrants should note that unless stated otherwise, GSK does not accept responsibility for the return of any Entries, including those consisting of artistic or other material.

Prizes
  1. Winners will be selected at random from all valid entries received before the relevant closing date within 10 days of the relevant closing date specified in the Competition Notice by Promo Veritas, the independent promotional verification service. Winners will be notified by telephone or by post within 3 days of the draw date. Those not contactable after reasonable efforts or who fail to return contact within a further 14 days will be disqualified and a reserve entrant selected on the same basis. No correspondence or discussion shall be entered into.
  2. Return of any prize notification as undeliverable or failure to reply as specified in the notification may result in disqualification and selection of an alternate winner. If more than one prize is awarded only one prize per entrant will be awarded. The names of competition winners will be available at www.promoveritas.com/dancingonice from 1 June until 1 July or you can write to the address set out in the Competition Notice naming the specific competition reference Dancing On Ice 2012 including a self-addressed stamped envelope for a list of winners.
  3. Failure to claim a prize within this time or in the manner specified may result in disqualification and selection of an alternate winner.
  4. Prizes are non-transferable and there is no cash alternative. GSK reserves the right to substitute prizes of equal or greater value at any time.
  5. Prizes are awarded at GSK ’s discretion and no prizes will be awarded as a result of improper actions by or on behalf of any entrant.
  6. All taxes, insurance, transfers, spending money and other expenses (including meals or personal expenses, upgrades etc) as the case may be, unless specifically stated, are the sole responsibility of the prize winner.

Intellectual Property Rights & Use of Entries
  1. In consideration of GSK agreeing to consider entrants to the Competition, each entrant hereby agrees that GSK (and third parties authorised by GSK ) may make any and all Entries available on the website and any other media, which may include other internal sites, mobile, television and/or radio and that Entries may be made available with advertising and/or sponsorship. You grant GSK (and third parties authorised by GSK ) a non-exclusive, worldwide, irrevocable licence (for the full period of any rights in the Entry) to use, display, publish, transmit, copy, make derivative works or podcasts from, edit, alter, store, re-format, sell and sub-licence the Entry for such purposes.
  2. Your Entry and any information submitted by you must be personal to and relate specifically to you. You hereby warrant that your Entry and all information which you submit and/or distribute will not infringe the intellectual property, privacy or any other rights of any third party, and will not contain anything which is libellous, defamatory, obscene, indecent, harassing or threatening. If relevant, GSK reserves the right, but not the obligation, to screen, filter and/or monitor information provided by you and to edit, refuse to distribute or remove the same.

Liability
  1. GSK cannot accept any responsibility for any damage, loss, injury or disappointment suffered by any entrant entering the Competition or as a result of accepting any prize. GSK is not responsible for any problems or technical malfunction of any telephone network or lines, computer on-line systems, servers, or providers, computer equipment or software, failure of any email or entry to be received on account of technical problems or traffic congestion on the internet, telephone lines or at any web site, or any combination of these, including any injury or damage to entrant’s or any other person’s computer or mobile telephone related to or resulting from participation or downloading any materials in the Competition. Nothing shall exclude GSK’s liability for death or personal injury as a result of its negligence.

Disclaimer
  1. This competition is being run by GSK Consumer Healthcare. ITV and its affiliates and parent company shall have no responsibility whatsoever with the running of the competition, and shall not be liable for any losses, damages or costs should an entrant make any claim against by GSK Consumer Healthcare as a result of the running of the competition

Data Protection and Publicity
  1. Winners may be requested to take part in promotional activity and GSK reserves the right to use the names and addresses of winners, their photographs and audio and/or visual recordings of them in any publicity.
  2. Any personal data relating to entrants will be used solely in accordance with the current UK data protection legislation and will not be disclosed to a third party without the individual’s prior consent. Data relating to entrants will be retained by GSK for a reasonable period after the Competition closes to assist GSK to operate competitions in a consistent manner and to deal with any queries on the Competition.

Governing Law
  1. The Competition and Rules will be governed by English law and any disputes will be subject to the exclusive jurisdiction of the courts of England.
